Septic Evaluation Request Form

Please fill in the necessary information below and Woody’s will be in communication.

As part of the Septic Evaluation, Woody’s staff will visit the property, dig PERC holes, complete all necessary forms, submit the paperwork to the county, pay the processing fee (included on your invoice), receive county reply, and send the results/invoice to email address specified on this form.

 

Additional Fees will be added if staff is unable to access property and a permit to clear is required prior to evaluation.
